ETHP Collaborate Terms of Use

 

ETHP Collaborate is a shared system developed by the ETHP used by ETHP members in respect of ETHP programs and initiatives.to support the delivery of integrated and collaborative care to community members. 

In using ETHP Collaborate you acknowledge that:

  • You have authority to share the client information through ETHP Collaborate.
  • The information you input to the system is complete and accurate to the best of your understanding.
  • You have received privacy training and instructions on how to protect client information and that this training applies to your use of ETHP Collaborate.
  • You will maintain the security of ETHP Collaborate. You will not share your password or log-in credentials, access ETHP Collaborate in public locations, or access the system over public Wi-Fi. 
  • You will only view information necessary to do your job. 
  • You will only access the minimum amount of client information that you need to provide care to that client or as permitted by the relevant ETHP program.
  • You agree to respect and protect confidential information in the system and will not disclose to any third party. 
  • If you print or copy any confidential information from within the system, you must keep it private and secure and permanently destroy it once you are done with it.
  • Your use of the system will be monitored and audited and consent to that.
  • You will not upload any documents to the “Uploaded Files” tab of the solution that have not been deemed to be acceptable for upload in the solution.
  •  You will comply with the policies and procedures associated with the relevant ETHP program when accessing ETHP Collaborate.
  • You will maintain the security of ETHP Collaborate including, not sharing your password or log-in credentials, not accessing ETHP Collaborate in public locations, or over public Wi-Fi Etc.
  • You are aware of and will comply with the policies and procedures of ETHP and the ETHP organization acting as Health Information Custodian of the program for which you are accessing the solution. The Health Information Custodian for each ETHP program that is currently using ETHP Collaborate is listed below: 
    • Michael Garron Hospital is the Health Information Custodian for the Primary and Community Care Response Team (PCCRT) Program. Termination and Restriction of Access

You acknowledge that if you breach these obligations your access to and use of ETHP Collaborate may be terminated or restricted and you may face other consequences.
